Primary care is the foundation of lifelong health. At Green Health Collective, you build a lasting relationship with a provider who understands your medical history, lifestyle, and goals. That understanding helps us tailor your care to you.
We focus on prevention, early detection, and ongoing care for acute and chronic conditions. Screenings, immunizations, and other preventive services are guided by your age, family history, and individual risk factors. This approach helps us identify and address potential concerns early.
We support adolescents as they transition from pediatric to adult primary care. Care is available for patients ages 16 and older following an initial provider consultation.
Whether you need a sick visit, help managing a chronic condition, or guidance about your health, we provide compassionate, comprehensive care. Same-day appointments are available whenever possible.
