Green Health Collective partners with Community Medical Group (CMG) and Privia Health

Green Health Collective has joined Community Medical Group (CMG) and Privia Health as part of the Privia Quality Network of Connecticut. CMG is Connecticut's largest independent physician network. Privia Health supports independent practices with technology, population health resources, and payer relationships.

Christa remains the sole owner of Green Health Collective and continues to make decisions about your care. The partnership provides additional support for the practice while preserving its independence.

As part of this transition, our electronic health records and patient portal have moved to athenahealth. The Healow portal has been retired. Please use the new Athena portal to message our office, request appointments, view records, and request prescription refills. Thank you for your patience during the transition.

Certified WBENC — Women's Business Enterprise

Green Health Collective receives national certification as a Woman-Owned Small Business

Green Health Collective has received national certification as a Woman-Owned Small Business through the Women's Business Enterprise National Council (WBENC), the country's largest third-party certifier of women-owned businesses. WBENC certification confirms that a business is at least 51% owned, operated, and controlled by a woman.

The certification process includes a detailed review of ownership, governance, and financial records, along with a site visit by a WBENC regional partner. Christa completed this process by providing the practice's formation, licensing, and operating records.

This recognition connects Green Health Collective with a national network of women business owners and WBENC's educational programs. We are proud to be an independent, women-led practice focused on our patients and community.

Green Health Collective celebrates its 1,000th patient appointment

This month, Green Health Collective celebrated its 1,000th patient appointment. We care for adolescents transitioning to adult primary care and adults seeking support with hormone health, weight management, and healthy aging. Many patients discover the practice through recommendations from friends, family, and other patients.

Our 5-star Google rating reflects the experiences patients have shared about their care. Thank you for trusting us with your health and recommending the practice to others. We look forward to continuing to care for this community.

Green Health Collective joins the Amity and Hamden Chambers of Commerce

Green Health Collective has joined the Amity Chamber of Commerce and the Hamden Regional Chamber of Commerce. The Amity Chamber supports businesses in Orange, Woodbridge, Bethany, and nearby communities through advocacy, networking, and professional development.

The Hamden Regional Chamber has connected local businesses with opportunities since 1925, supporting economic growth and a strong business community. Our membership in both chambers reflects our commitment to the communities where our patients live and work.

Green Health Collective opens

Christa Mann opened Green Health Collective in Woodbridge, Connecticut, on September 9, 2024. She founded the practice to offer personalized primary care, with time to listen and an understanding of each patient's overall health.

Services include hormone health, weight management, preventive screenings, and support through different stages of life. As an independent, provider-led practice, Green Health Collective gives Christa the freedom to make evidence-based care decisions around each patient's needs and preferences.

The practice welcomes adolescents and adults, with in-person and telehealth visits available. Same-day or next-day scheduling is often possible. Thank you to everyone who has supported the practice from the beginning.
