We focus on prevention, early detection, and ongoing care for acute and chronic conditions. Screenings, immunizations, and other preventive services are guided by your age, family history, and individual risk factors. This approach helps us identify and address potential concerns early.
We support adolescents as they transition from pediatric to adult primary care. Care is available for patients ages 16 and older following an initial provider consultation.
Whether you need a sick visit, help managing a chronic condition, or guidance about your health, we provide compassionate, comprehensive care. Same-day appointments are available whenever possible.
A typical visit includes a review of your health history and current concerns, a physical exam when needed, and a discussion of any lab work or screenings that are due. You will leave with a clear plan, which may include a prescription, referral, or follow-up appointment.
Primary care at Green Health Collective connects with mental health support, hormone health, weight management, and other services. When a concern involves more than one area of health, Christa can address it directly or coordinate a plan across services. Your care builds on the history you have already shared with us.
We welcome adolescents and adults. Same-day or next-day scheduling is often available. Complete the intake forms requested for your visit in advance to save time at check-in.
We welcome new primary care patients. We also accept referrals for hormone and menopause care, weight management, and the evaluation, care, and coordination of lymphedema and lipedema. School and sports physicals are available, and same-day or next-day scheduling is often possible.
